Online Application
You
can now use an online application to apply for Medical Assistance and
MinnesotaCare. The online application
can also be used for SNAP (Food Support), Child Care Assistance and some other
state-sponsored programs. To apply
online, go to ApplyMN.
Paper Application
If
you prefer to use a paper application, you can download one below.
MinnesotaCare and Medical Assistance use the same application. Once you have completed the paper application, you can mail it or deliver it by hand to your county's or tribe's human services office.
If you don't know the address of your county human services office, you can call 651-431-2670 (Twin Cities) or 1-800-657-3739 (Greater MN), click here to use the Program Directory on this website or click here to find it on the Minnesota Department of Human Services' website. The
application is available from your
county human services office or by calling the
